Parent Permission and Physical Form

2023 Wrestling Season Starts October 09, 2023.  October 09-13 practices will be held at Black Canyon MS from 4-6pm.  

In order to participate students must have a current physician signed physical on file, wrestling parent permission form turned in by October 6, 2023. Forms are available in the front office.  Pay to play for wrestling is $50.00 and all students will need to have an activity card, if they do not they can be purchased from the School Bookkeeper for $15.00.
