Students who are participating in Track will need to fill out the Middle School Extracurricular Participation Form and submit a physical and a signed parent consent form.

Parents/guardians need to upload a doctor’s physical digitally & digitally sign the consent forms. If Black Canyon MS does not have a physical on file or a consent form, students will not be allowed to participate without proper documentation.
Participation fee for track is $100.00 and all students will need to have an activity card, if they do not they can be purchased from the School Bookkeeper for $15.00. Students are also required to purchase a jersey for $20 if they do not own one.
